A PATIENT CENTRED SERVICE

All children, young
people and adults with Chronic Kidney Disease are to have access to information
that enables them wither their carers to make informed decisions and encourages
partnership in decision-making, with an agreed care plan that supports them in
managing their condition to achieve the best possible quality of life.
Using this standard
as a benchmark and working in collaboration with the multi disciplinary team, a
comprehensive pre-dialysis education programme was developed which encompassed
the biopsychosocial needs and well being of patients and their carers.
A named contact who
helps patients navigate the renbal pathway is identified. Their role is to
ensure that negotiated goals are achievable and attainable. An agreed care plan
encourages free discourse that fosters a collaborative partnership and promotes
self-management.
Patient awareness days were implemented to run
alongside the more formal education programme. Meeting members of the multi
disciplinary team ensures patients and families have access to appropriate
material in a safe format, maximizing their learning potential. Patient centred
meetings encourage the development of social support and group networking at
the beginning of their dialysis career. This inspires patients to become full
partners in care and to share responsibility for managing their own condition.
This presentation
aims to demonstrate that through regular evsaluation, the patient awareness
dats have evolved from passive to a collaborative approach in which patients
are active participants with shared responsibilities. The objective is to
strive towards building a firm foundation of knowledge and confidence to
faciliate a better quality of life.
